New Patient Registration

If you live in our catchment area, please call to make an appointment to register at the practice

Practice Catchment Area

To register you must be eligible for NHS care and be able to confirm that you live within the catchment area. A map is available at the reception desk. A recent household bill and photo identification( e.g. passport or photo driving licence) is needed to register. As part of the registration process you must complete a form and a health questionnaire. Both are available at reception.

The practice does not discriminate against patients on grounds of ethnic origin, religious belief, disability, gender or on the nature of your health problems.

Medical treatment is available from the date of registration.  Please contact reception for further information.

Moving House?

Please inform us if you change address or move out of our catchment area. You will need to register with a new GP as we will no longer be able to provide a service to you. If this occurs you will be sent a removal letter and asked to register elsewhere.
