National Care Summary Record

  The Summary Care Record is an electronic record which will give healthcare staff faster, easier access to essential information about you, to help provide you with safe treatment when you need care in an emergency or when your GP practice is closed.


 


A Summary Care Record (SCR) is an electronic summary of a patients’ key health information which those treating you can get access to anywhere in England. This will only include 3 basic details:


 


·                     Allergies


·                     Current Prescriptions and


·                     Reactions you have to medications


 


If you choose to have a Summary Care Record, then this will happen automatically. However, if you do not wish to have a Summary Care Record created then you are able to “opt out” by filling in an “opt out” form which is available by visiting http://www.nhscarerecords.nhs.uk/havescr or within the surgery.


( Please note if you opted out when first contacted about this in 2009/2010, your decision still stands and is recorded in your medical records)
